Rezidivtherapie des Multiplen Myeloms
Zusammenfassung
Die Prognose von Patienten mit Multiplem Myelom konnte in den letzten Jahren durch eine stetige Vergrößerung des Arsenals zugelassener Substanzen signifikant verbessert werden. Insbesondere in der Rezidivsituation stehen dem behandelnden Hämatologen/Onkologen mittlerweile eine Vielzahl an Behandlungsoptionen zu Verfügung, sodass eine zunehmend individualisierte Therapiegestaltung ermöglicht wird. Substanzen der nächsten Generationen von Immunmodulatoren und Proteasominhibitoren wie Pomalidomid und Carfilzomib zeigen höhere Ansprechraten bei verbessertem Nebenwirkungsprofil, Kombinationstherapien aus Immunmodulator und Proteasominhibitor können adäquate Remissionen auch bei aggressivem Krankheitsverlauf erzielen. In der refraktären Situation eröffnen Entwicklungen in der Immuntherapie mit den kürzlich zugelassenen monoklonalen Antikörpern Elotuzumab und Daratumumab neue Möglichkeiten.

Treatment of relapsed multiple myeloma
Summary
The prognosis of patients with multiple myeloma has significantly improved in recent years due to the steady increase in the armamentarium of anti-myeloma therapeutic agents. Especially in the relapsed setting, the treating hematologist can now choose from many different treatment options enabling an increasingly personalized therapy. Next-generation immunomodulatory agents and proteasome inhibitors, such as pomalidomide and carfilzomib, show deeper responses with lesser side effects, combination therapy of immunomodulatory agents and proteasome inhibitors can achieve adequate responses even in highly aggressive disease. Developments in the field of immunotherapy only starting with the recently approved monoclonal antibodies elotuzumab and daratumumab open up completely new therapeutic options.
